Roadmap to 5-Star Wins is a focused webinar series designed to help skilled nursing facilities improve performance across all CMS 5-Star domains. Each session breaks down key components—Health Inspections, Staffing, Quality Measures, and QRP—into clear, actionable strategies. Participants will learn how ratings are calculated, how data accuracy and operations impact scores, and how to build a customized, sustainable plan for achieving stronger 5-Star outcomes.
March 11: Building Your Customized Work Plan for 5-Star Success
This capstone session brings together all elements of the 5-Star domains and guides attendees through building a customized facility improvement plan. Participants will learn how to analyze performance trends, set achievable goals, and apply QAPI principles to sustain improvement. Templates, checklists, and real-world examples will help guide their action planning.

1. Interpret facility data to identify performance gaps.
2. Develop measurable, domain-specific improvement goals.
3. Create a stepwise, interdisciplinary work plan aligned with QAPI principles.
